Shift Details:
Monday - Friday 8:00AM - 4:30 PM
Perform comprehensive nutrition screenings and assessments.
Develop and implement individualized Medical
Nutrition Therapy(MNT) plans.
Document nutrition assessments, interventions, and outcomes within the electronic medical record.
Provide nutrition groups, education and counseling to patients, families, and caregivers. Participate in facility-wide and
departmental Quality Assurance and Performance Improvement initiatives.
Participate in multidisciplinary patient care rounds and departmental meetings.
Support staff development initiatives and contribute to improved clinical outcomes.
Complete Swallowing Screen as needed.
Minimum Qualifications:
Current, valid, and unrestricted NY license/certification in good standing, as required for the position.
Bachelor's or Master's Degree in Clinical Nutrition, Dietetics or a related field from an accredited institution
Registered Dietician(RD) or Registered Dietitian Nutritionist Credential.
Minimum of one (1) year related experience.
Additional certifications may be required.
Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
